Incomplete forms will not be accepted. Gift cards will be given ONLY to those who fill out this form and submit it by October 1st of the current school year. Only one gift card will be given for each new student. If two forms are received, the first one submitted will be honored. Gift cards cannot be split. CECFC cannot give referral gift cards for siblings of current or former students, or students living in the same household as the referring person. Gift cards will be issued in December and must be claimed before the deadline specified.
